Department of Aviation and Technology Student Forms
for B.S. Aviation and B.S. Industrial Technology majors, minors,
New Beginning Fall 2009! Declaring your major in Industrial Technology or in AviationIt is very important to declare your major as soon as you are enrolled in courses for your degree, if not before. This sets up a file in the department office and also gives you and the department recognition of your advising and enrollment. If you have not yet declared your major, your major will appear in your MySJSU account as "undeclared" or as in another department. Please download and complete the "Change of Major Form" (link below) or pick up a copy in the office, and bring it to the department office for copying and for the chair's signature. If you have not done so yet, you will also want to meet with a departmental advisor and review your program progress and requirements. Faculty contact information is here. Change of Major or Minor Form (link to Academic Forms page) Aviation and Industrial Technology Academic Minor FormIf you wish to obtain a minor in either Aviation or Industrial Technology, you should submit an academic minor form (link here) to a department advisor for a signature. After you receive the signature of the advisor, you should submit the form to IS 111 for the department chair's signature. InternshipsDepartment of Aviation & Technology students can enhance their class experiences with aviation, industrial, or business internships. You can download more information about internships by clicking on the internship information link below. To enroll in an internship you will need a faculty advisor and an industry position to work in. Your advisor may be able to help you locate a suitable internship. If you want to apply for an internship, you should click on the internship forms link. Both of these linked documents are in Adobe Acrobat format. Visit the Department Internship page for more information on Internships. Individual Studies coursesDepartment of Aviation & Technology students can enhance their class experiences with an independent study once they achieve upper division standing. The purpose of an independent study is to allow you to study topics or materials that are not covered in regular courses. These usually carry the course number Tech180 or Avia180 and can be for from 2 to 4 semester units of credit. You begin by contacting a faculty advisor who will supervise and work with you on your independent study. If you want to apply for an independent study, you should click on the independent study form link. This form is in Microsoft Word format.
